We understand how important it is to keep your child safe and healthy at school—especially when dealing with food allergies or medical conditions. Our updated menu website makes it easier than ever to find meals that meet your child’s needs.

🧾 Need a Special Menu?
If your child has a disability or life-threatening food allergy and requires a menu that deviates from the USDA meal pattern, you’ll need to submit a Disability/Medical Condition Statement for Foods Served.

Here’s how to get started:

  • 📞 Contact the school nurse or health associate where your child attends.

  • 📞 Or call the Nutrition Services Department at (563) 663-9473.

  • 🖨️ You can also print the form from the Related Documents section below.

✅ The form must be signed by an Iowa state-licensed physician (MD or DO), a Physician Assistant (PA), an Advanced Practice Care Nurse, or a Registered Dietitian Nutritionist and returned to your child’s school nurse. Registered Dietitians signing a special diet statement should be part of the participants/ healthcare team.

📝 If No USDA Menu Deviation Is Needed
If your child’s dietary needs can be met within the USDA meal pattern, you do not need the above form. However, you will still need documentation signed by a licensed medical practitioner.
